I understand that submitting this form does not give me entitlement to a place on a waiting list or adopting a puppy. If there is room on the waiting list, and when puppies are available, and upon approval of my adoption application, I(we) will pay a deposit or forfeit my place and or selection of the puppies. Upon approval I understand it is my responsibility to stay in contact with the breeder and keep my contact info current in order to secure my place in line for a puppy. Until I make a deposit I understand that the breeder reserves the right to refuse service to me (us) at any time for any reason. *
